Respite Services, Personal Care, and Camp Available
Easterseals is excited to offer the Agency With Choice (AWC) to individuals enrolled in the Medicaid Lifespan Waiver through the Division of Developmental Disabilities Services!
If you live in your own private residence or in the home of a family member, Easterseals can help you access up to $5,000 a year to use as you need through the Agency with Choice program.
With a referral by your Community Navigator, our Agency With Choice Coordinators can help you recruit, hire, train, and supervise your caregivers! You may choose to use your funds for respite services, such as Camp, or for personal care.
Easterseals AWC coordinators can answer your questions and assist you with enrollment paperwork. Your $5,000 annual allotment can be used to pay the caregiver that you choose or to pay to attend respite camps, as approved by your Community Navigator.

Contact your Community Navigator and ask for a referral so Easterseals can start to serve you and your loved one. Once you have the referral, you can start receiving Agency With Choice benefits through Easterseals.
